Nominated Beneficiary Information

This screen allows you to view your nominated beneficiary information and to ensure that it is correct. Be sure to read all information carefully. A nominated beneficiary is the person who has been named to receive your benefit on the occurrence of your death. Your nominated beneficiary may also be referred to as your "designated" beneficiary. This screen displays your primary and secondary, or alternate, nominated beneficiaries as they are on file with the retirement office.

To access your nominated beneficiary information, select the Nominated Beneficiary option in the left pane of the window. The Nominated Beneficiary screen appears.

When the Nominated Beneficiary screen opens, your Primary and (if applicable) Secondary Beneficiaries are displayed.

Information for each beneficiary includes:

• Date of Birth. This is the nominated beneficiary's date of birth.
• Benefit Type. This identifies which of your benefits the nominated beneficiary is associated with.
• Survivor Type. This describes the benefit to be continued to the survivor.
• Designation. This is the beneficiary designation (Primary or Alternate).
• Designation Percentage. This displays the percent of your benefit that the beneficiary will receive if you should die prior to retirement. For example, you may want to designate 100% to a primary, or 50% to a primary beneficiary and 50% to a secondary beneficiary.
